HANDOVER Reaches Out to Health Care Providers Worldwide

The author of hundreds of academic papers, Dr. Paul Barach has written medical articles published in New England Journal of Medicine, Journal of the American Medical Association, British Medical Journal, Academic Medicine and many more. Dr. Paul Barach supports a number of international organizations and programs, including HANDOVER project.

HANDOVER is a project dedicated to ongoing improvements in patient care and to improving patient discharge and transition to the community. The project focuses primarily on the continuity of treatment. Continuity in care becomes important and potentially lifesaving when a primary physician refers a patient to a nearby hospital or a specialist, but the transition may be handled poorly or incompletely. HANDOVER works to promote more effective, patient and community centred patient transitions and handovers.

Over the past few years, HANDOVER has organized a series of expert meetings in cities all over the world, including in Florence, Amsterdam, Brussels, and Berlin. These meetings address patient handover and transition culture issues, the costs of proper patient transition, and the effects of improper handover on the patient, family and the healthcare system.
